  1. primary atrophic hypothyroidism - most common cause
  2. Hashimoto’s thyroiditis
  3. postpartum thyroiditis
  4. iatrogenic - thyroidectomy or radioiodine treatment
  5. drugs - amiodarone, carbimazole, lithium, iodine
  6. iodine deficiency - chief cause worldwide

A

associated with antithyroid autoantibodies / leading to lymphocytic infiltration of the thyroid / CD8+ T-cell mediated destruction / with atrophy and fibrosis / no goitre formation

A

thyroid peroxidase antibodies (TPO-Ab) present in high titres / lymphocytic and plasma cell infiltration / atrophic changes with regeneration / goitre formation

anti-arrhythmic drug can cause hypothyroidism as it inhibits the conversion of T4 to T3 / can also cause hyperthyroidism due to high iodine content

lifelong thyroid hormone replacement / oral levothyroxine (T4) / caution in elderly patients or those with ischaemic heart disease as may precipitate angina or MI

How is treatment monitored? [3]

A
  1. aim is to normalise but not suppress TSH
  2. primary hypothyroidism - dose titrated until TSH normalises / check T4 levels 6-8 weeks after dose adjustment
  3. secondary hypothyroidism - TSH will always be low / but T4 is monitored

severe hypothyroidism (reduced T4) / the ultimate hypothyroid state before death

  1. hypothermia
  2. hypoglycaemia

How is myxoedema coma treated? [4]

A
  1. glucose infusion - treat hypoglycaemia
  2. IV liothyronine (T3) - replace thyroid hormone
  3. IV hydrocortisone - until adrenal insufficiency has been ruled out
  4. active warming - treat hypothermia
