Myedema coma

A

When hypothyroidism left untreated

Features:

  • Looks hypothyroid
  • Hypothermia
  • Hypoglycaemia
  • Heart failure: bradycardia and ↓BP
  • Coma and seizures

Causes of hypothyroidism

A
Atrophic thyroiditis 
Hashimoto’s thyroiditis
Iodine deficiency - simple goitre
Drugs: carbimazole, amiodarone, lithium
Congenital: thyroid agenesis 

Post surgical: after radio-iodine or thyroidectomy

Atrophic Thyroiditis

A

Thyroid antibodies +ve: anti-TPO, anti-TSH
Lymphocytic infiltrate → atrophy

Associations:

  • Pernicious anaemia
  • Vitiligo

A
Bloods: TFTs, FBC, U+E, glucose, cortisol
Correct any hypoglycaemia
T3/T4 IV slowly
Hydrocortisone 100mg IV
Tx hypothermia and heart failure

A

Associated with decreased iodine uptake
Diffuse painful goitre
Preceding viral URTI common - Coxsackie virus
Thyrotoxicosis → hypo→ eu

Tx - self limiting

Side effects of thyroxine

A

Worsens angina
AF
Reduced bone mineral density
Hyperthyroidism

Give levothyroxine at least 4 hours apart from calcium or iron due to decreased absorption
