Hyperlipidemia Flashcards
If statins are used in adults greater than 75 what intensity is used?
Moderate intensity
What medication should be used with caution in the geriatric population?
Bile acid resins due to hydration status. Dehydration can lead to constipation and fecal impaction.
What medication is safest in pregnancy?
Bile acid resin but be careful because can block nutrients to baby.
Statins are what category of medication?
HMG-CoA reductase inhibitor
MOA of a stain?
Blocks production of cholesterol in the liver. Decreases LDL in the bloodstream. Also leads to a decrease triglyceride levels and modest increases on HDL.
What pregnancy category are statins?
X
When to give statins?
At night due to increased cholesterol production and bioavailability
What intensity statin in group with clinical ASCVD?
High intensity <75
Moderate intensity >75
What intensity statin is given to group with no ASCVD but LDL greater than 190?
High intensity
What intensity statin is given to group with no ASCVD, 40-75, with diabetes, and LDL 70-189
Moderate if 10 year ASCVD risk <7.5
High is 10 year ASCVD risk >7.5
What statin intensity is given to group with no ASCVD or diabetes, 40-75, LDL of 70-189 and have a 10 year ASCVD risk >7.5.
Moderate intensity
What do fibric acid derivatives mainly effect?
Triglycerides and HDL levels. Do not decrease LDLs.
Bile acids MOA:
Bind to bile acids in intestines, forming an insoluble complex that is excreted in the feces. This decreases the return of cholesterol to the liver.
Use bile acids with caution:
In patients with fasting triglyceride levels between 250-299. Do not use in patients with fasting triglycerides greater than 300 bc they can increase triglycerides up to 15%.
How do you take bile acids?
With meals since they block absorption of cholesterol from GI tract.
