HYHO HYs Flashcards
What are sx of hypoK?
muscle weakness
parasthesias w tetany
HA
polyuria/dipsia
Dec levels of what are consistent with GH def?
IGF1 and IGFBP3
If ^ are decreased, GH stimulation test should be done:
- administer glucagon
- adminster arginine
- insulin induced hypoglycemia
What are the growth parameteres on growth charts?
height or length
weight
head circumference
3 causes of beign short stature
- Constitutional - late bloomer, bone age is delayed
- Familial - bone age matches actual age
- Idiopathic - bone age matches actual age
if a pt has low renin HTN, you should think..
*primary aldosteronism
low renin HTN
hypokalemia
metabolic alkalosis
How do you tx a pt in acute adrenal crisis?
hydrocortisone
glucose/fluids
fludrocortizone (after hydro given)
furosemide (tx hyper K if needed)
Elevated midnight cortisol levels indicate…
dx of CS
what is the most sensitive test for dx of pheochromocytoma/paraganglioma?
plasma fractionated free metanephrines
When do you excise an adrenal incidentaloma?
>4cm and obvi not benign
hx of cancer
Why/when do you give the low dose of dexamethasone?
at night to see if you get supression of cortisol (labs drawn next morning)
–> if you get supression, CS is excluded
How do you tx an infant in crisis due to CAH?
Hydrocortisone (IV or IM)
glucose/fluids (IV)
management of hyperK
What lab tests are ordered for suspected precocious puberty?
LH, FSH< Estradiol/Testosterone, 17 hydroxyprogesterone
What lab findings are seen in acute adrenal crisis?
low cortisol
low blood sugar
low sodium
high potassium
metabolic acidosis
ACTH stimulation test –> inadquate inc in cortisol
