How does CHG differ from soap and water baths?
- CHG allows for less bacterial growth in basins
- Disposable and no basins needed
Complete bed bath steps:
Verification, Supplies, Introduction:
- Verify orders
- Gather supplies
- Introduce yourself to pt and family
PT Identification and Discussion:
- Identify with 2 identifiers (Name and B-Date)
– Compare info with pt bracelet
- Explain procedure to pt and see if they agree
Assessment and First Steps:
- Assess pt for:
– Activity tolerance
– Shortness of breath
– Comfort level
– Cognitive ability
– Musculoskeletal function
– Skin condition
- Encourage pt to void and offer bedpan or urinal if needed
- Preform hand hygiene and put on clean gloves
- arrange supplies on bedside table
- raise bed to comfortable height
- Lower side rail on nurse side and put pt in a comfortable supine position
- move pt toward nurse and place a bath towel/blanket over them
– ask them to hold top of towel/blanket while you remove the top sheet from under the towel w/o exposing pt, then place used linens in dirty bag/container - remove pt gown or pajamas, and remove exposed parts with towel
- if possible, remove pillow and raise head of bed 30-45 degrees
- place a towel under pt head and one over chest
eyes, face, ears, nose
- check for contact lenses
- clean eye going inner to outer areas
- ask pt if they want soap on their face or if they prefer warm water
- wash rinse and dry pt forehead, cheeks, nose, neck, and ears
- avoid tension on any areas w/ tubing
- provide skin care around nose and ears
- ask pt to remove hearing aids
- if a pt is unable to remove nose secretions w/ tissue, use a wet washcloth or cotton-tipped item moistened in water/saline (never go further than cotton tip)
CHANGE WATER/SOLUTION
Upper Extremities
- place towel under closest arm, lengthwise
- bathe extremities w/ long, firm strokes and moving proximal to distal to promote venous return
- Dry extremities and each finger individually
- when washing armpits, raise arms above head if possible
- apply deodorant if needed
Chest/trunk
- fold towel down to umbilicus
- use long firm strokes moving side to side
- for women, clean under breast tissue
Abdomen
- fold towel down to just above the pubic region
- apply clean gown afterwards
- pay special attention to umbilicus and surrounding skin folds
Lower extremities
- cover chest/abdomen and legs w/ towels
- place towel lengthwise under leg while supporting knee and ankle
- go ankle to knee, then knee to thigh, then clean feet and toes well
