A
Corticosteroid, anti-inflammatory

Mechanism of Action
A
- Replaces the naturally occuring hormone, cortisol, produced in adrenal cortex
- Minimizes allergic response by inhibiting histamine formation, storage, and release from mast cells

Indications
A
- Allergic reactions
- Severe anaphylaxis
- Asthma
- COPD

Contraindications
A
- Hypersensitivity

Adverse Effects
A
- Fluid retention, CHF, HTN, tachycardia
- Vertigo, headache
- Abdominal distention, nausea, vomiting, hiccups
- Malaise, petechiae

Administration
A
- Adult: 40-250 mg SIVP at 25 mg/min; or deep IM
- Peds: 4-8 mg/kg/day SIVP; or deep IM

Special Considerations
A
- When indicated there are few precautions for acute prehospital emergency care
- Not a first line drug. Considered an adjunct.
- IV route preferred in emergency care
- Supplied in vials. Powder must be reconstituted in a diluent. Do not shake vial.
- Caution with CHF, HTN, diabetes, or kidney disease
