hydro Flashcards
self management involves
- Disease management – i.e. treatments (medical and nonpharmacological), behavioural interventions
- New roles – what will change in their life?
- Psychological factors – emotions come along for the ride
self management in chronic disease includes
- Learning about the chronic health condition
- Partnership with practitioner, shared decision making
- Skills development
- Peer support
barriers to self management
- Limited partnership with health care practitioner(s)
- Limited access to self management programs and health care
resources - Low health literacy
what facilitates self management
- Building patients’ self-efficacy * i.e. “I can do this!”
- Asking about patients’ goals and challenges
- Shared group appointments/education sessions
- Access to self-management programs
treatment variables that impact intensity
temperature
duration
wetness
friction/pressure
size of applciation
frequency
time of day
permeability
modifying intensity in extreme temeprature and neutral temperature
extreme temperature –> short duration –> more intense
neutral temperature –> ling duration –> more intense
modify intensity by wetness
wetter= more intense
modify intensity by friction and pressur
more friction or pressure= more intesnse
modify intensity by size of application
increased area covered by heat –> stronger derivative effect
smaller area covered by cold –> stronger revulsive (depletion)/retrostatic effect
modify intensity by frequency
repeat treatment more often= more intense
best time of day for patient may enhance treatment effefct
variables that impact if effect is reflexive or direct
temperature (how far is it from body temeprature)
location
reflexive is useful when
limited access to area
local hot or cold contraindicated
what carries temperature well
water
closed system
Skin > nerves > vessels
how to direct blood and lymph flow
Derivation
derivation- what is the mechanism?
- Heat is the mechanism
- Blood and lymph flow towards heat
depletion (revulsive/retrostatic)- what is the mechanism?
- Cold is the mechanism
- Blood and lymph flow away from cold
alternating/ contrast
- Combination of heat and cold = pumping action
- Decongestive
arterial trunk reflex
Application over arterial trunk has the same effect on arterial branches
collateral circulation
- Relationship between superficial and deep branches of a vessel
- Opposite effect between branches
if apply to one branch (superficial) heat and vasodialte, then the other branch (deep) will constrict
modify intensity by permeability
Covering material more impervious–> increased derivative effect
burnout
“… chronic workplace stress that has not been successfully managed. It is characterized by three dimensions:
- feelings of energy depletion or exhaustion;
- increased mental distance from one’s job, or feelings of negativism or
cynicism related to one’s job; and * reduced professional efficacy” - Specifically related to work, not in other areas of life.
- Not a medical condition
compassion fatigue
“… the cost of caring for others or for their emotional pain, resulting from the desire to help relieve the suffering of others.
It is also known as vicarious or secondary trauma, referencing the way that other people’s trauma can become their own.”
signs of compassion fatigue
Feelings of helplessness and powerlessness in the face of patient suffering
* Reduced feelings of empathy and sensitivity
* Feeling overwhelmed and exhausted by work demands
* Feeling detached, numb and emotionally disconnected
* Loss of interest in activities you used to enjoy
* Increased anxiety, sadness, anger and irritability
* Difficulty concentrating and making decisions
* Difficulty sleeping and sleep disturbances like nightmares
* Physical symptoms like headaches, nausea, upset stomach and dizziness
* Increased conflict in personal relationships
* Neglect of your own self-care
* Withdrawal and self-isolation
* Increase in substance use as a form of self-medication
general themes for burnout and compassion fatigue
- Boundaries (where possible)
- Self-awareness
- Recognizing limitations, control
- Can self-compassion help?
symptoms are similar in burnout and compassion fatigue
- Symptoms are similar
- Burnout related to work,
- Compassion fatigue in helping professions
- Similarities in addressing both: * Boundaries
- Pause
- Possibly a role for self-compassion
informed consent
- Is a conversation and ongoing
- Is not sufficiently completed when someone signs a form
- For:
- taking the history
- completing physical exam
- Interventions (assessment and treatment) * collecting personal health information
- Should be repeated each time will repeat treatment, even if it is the same treatment
aspects of informed consent
- Nature of the intervention (or assessment) and rationale
- Expected outcomes and/or benefits
- Material risks (including financial costs, significant harm or death)
- Alternatives
- Consequence of no treatment
- Right to withdraw consent at any time
capacity in informed consent
- Practitioner to determine capacity
- Needs to be able to understand information related to making the decision
and potential consequences of the decision - Presumed capacity unless have information to believe otherwise
