What are lapses
When a person forgets to do something
Such as forgetting to restock a truck
Why do mistakes, slips and lapses happen
As unintended actions with unintended consequences
Due to:
- mental workload
- distraction
- physical environment
- physical demands
- communication
- competence
What is PACE
An assertive technique for dealing with human errors
P- Probe (raise concern)
A- Alert (offer alternative)
C- Challenge (challenge decision)
E- Emergency (ask them to stop what they are doing)
