Uncomplicated HTN (no comorbidities)
Thiazide(prefered indapamide or chlorthalidone over HCTZ) Ace Arbs BB CCBs
Avoid:
-ace in african americans or pregnant,
-BB in pt under 60
ISH (SBP>= 140 DBP< 90)
TH ARB CCB(dihydropyridine)
HTN + DM (no albuminuria, CKD,CVD, OR CV risk factors)
Th Ace Arb CCB (dihydropyridine first line add on acei)
HTN +DM+ albuminuria (ACR>2mg/mmol) CKD,CVD, risk factor for CVD
ACE ARBS (dihydropyridine second line add on acei) consider loop in hypervolemia
HTN+ CAD
ACE ARB BB CCB (DHP best may consider other classes add on to acei)
HTN + Recent MI
ACE ARB BB (ccb consider both classes avoid non-DHP in HF and caution in combo of BB)
HTN + HF
ACE ARB BB
LV hypertrophy
TH ACE ARB CCB
History of TIA
TH ACE
CKD + Albuminuia (no DM)
ACE ARB
Pediatrics
ACE ARB BB CCB (BB less preferred)
PREGNANCY AND LACTATION (all females of child bearing age should have a pregnancy test done and drug therapy should be discussed prior to conception)
Methyldopa, LA nifiedipine, or Labetalol (or other BB except atenolol)
