DDx Perilymphatic
A
- Sarcoidosis (patchy, asymmetric)
- Silicosis (subpleural nodules are esp common, symmetric, posterior upper lobe predom)
- Other pneumoconioses: CWP, Berylliosis, Talc
- Lymphangitic spread of cancer (carcinoma, lymphoma)
- Lymphoproliferative disease (LIP, Sjogren’s, multicentric Castleman, AIDS)
- LIP can often mimic lymphangitic spread of cancer on HRCT - Amyloid, light chain deposition disease
