DDx Perilymphatic

A
  1. Sarcoidosis (patchy, asymmetric)
  2. Silicosis (subpleural nodules are esp common, symmetric, posterior upper lobe predom)
  3. Other pneumoconioses: CWP, Berylliosis, Talc
  4. Lymphangitic spread of cancer (carcinoma, lymphoma)
  5. Lymphoproliferative disease (LIP, Sjogren’s, multicentric Castleman, AIDS)
    - LIP can often mimic lymphangitic spread of cancer on HRCT
  6. Amyloid, light chain deposition disease
