Optic
Test visual acuity with snellen eye chart and opthalmoscopic exam of the eye and confrontation test.
Olfactory
With eyes closed, and one nostril occluded, ask patient to identify aromatic substance held under nose
Oculomotor
Test using PERRLA
Trochlear
Test using PERRLA
Abducens
Test using PERRLA
Trigeminal
Clench teeth, palate temporal and masseter muscles for muscle mass and strength. Light touch using cotton ball to touch anterior scalp, paranasal sinuses and jaw. Corneal reflex.
Facial
Inspect face and rest and during conversation. Raise eyebrows, purse lips, close eyes tightly, smile, frown, puff cheeks.
Acoustic
Rinne and weber’s test
Glossopharyngeal
Touch posterior pharynx with tongue blade, patient should gag. Have them say light, tight, and dynamite.
Vagus
Say “ah” look for equal movement of soft palate and uvula
Spinal
Shrug shoulders against resistance
Hypoglossal
Stick tongue out, look for symmetry, have them move tongue side to side with resistance from a tongue blade.
