In mild/moderate community acquired pneumonia, what antibiotics should be used?
Amoxicillin 1g tds for 7 days (PO/IV)
Doxycycline 200mg then 100mg od
In severe community acquired pneumonia, what antibiotics should be used?
Co-amoxiclav IV & clarithromycin IV or doxycycline
Levofloxacin if penicillin allergic
Step down to doxycycline in all severe CAP
In severe hospital acquired or aspiration pneumonia, what antibiotics should be used?
IV gentamicin & amoxicillin & metronidazole
Step down to co-amoxiclav
In non-severe hospital acquired or aspiration pneumonia, what antibiotics should be used?
Amoxicillin & metronidazole for 7 days
In an acute exacerbation of COPD, what antibiotics should be used?
1st line amoxicillin 500mg
2nd line doxycycline 200mg then 100mg daily for 5 days
In endocarditis, what antibiotics should be used in a patient with a native valve affected? (Subacute/non-severe)
Amoxicillin & gentamicin IV
In endocarditis, what antibiotics should be used in a patient with a native valve affected? (Acute/sepsis)
Flucloxacillin IV
In endocarditis, what antibiotics should be used in a patient with a prosthetic valve affected? (Same for suspected MRSA)
Vancomycin IV & Rifampicin 600mg PO bd & Gentamicin IV
In a non severe infection with C. difficile, what antibiotics should be used?
Metronidazole PO 400mg tds
In a severe infection with C. difficile, what antibiotics should be used?
Vancomycin 125mg qds (PO/NG)
consider adding IV metronidazole
In patients suffering from peritonitis/biliary tract/intra-abdominal infection, what antibiotics should be used?
IV amoxicillin & metronidazole & gentamicin
If penicillin allergic - IV vancomycin & metronidazole & gentamicin
