What is the purpose of history taking?
To collect accurate subjective information about the patients health status, to listen to the patients story, to assess for red flags and signs of deterioration, to establish rapport and trust, to identify any concerns and to help with care
How do you effectively take a patients history?
Have a two-way conversation with the patient, use a variety of communication skills, gain consent from the patient, introduce yourself and explain why you are gathering information, use appropriate structured approaches.
What information do you explore in history taking?
Presenting problem, past medical history, current medical history, allergies, personal and social history, family history, mental health, keep red flags in mind - history of certain illness etc
