Hip Special Tests Flashcards
iliofemoral ligament
medial/vertical
- pt prone
- PT IR, ext, and ABd hip while stabilizing posterior inominate
Lateral/horizontal
- Pt prone
- PR IR, ext, and ADd hip whil stabilizing posterior inmoniate
Pubofemoral ligament
pt supine
PT stabilize pubic bone while ABd and slight Ext
Ischiofemoral ligament
pt supine
PT stabilizes anterior inominate while flex, ADd, and IR the hip
Ligamentum Teres
Pt supine
PT holds leg in flex and ADd the leg
Trendelenberg Sign
pt on SLS ON AFFECTED LEG to test the correct side
hold for 30 seconds
look for any hip drop or weakness or unsteadyness
PT can lightly palpate crest of pelvis or PSIS
Pelvic drop
pt on 8in step and do unilateral squat
look for knee going into valgus, pelvid drop, femoral IR and pronation
good to do with shoes on and then barefoot
piriformis
pt supine with testing leg bent
PT puts compression force into hip through the knee and then again in ADd
FAIR test
pt sidelying
PT bring hip into passive flex, ADd, and IR
Thomas/Modified Thomas
- pt supine and they bring bent knee to their chest
look for flexion of the Lspine and lifting of other leg off the table - pt supine with legs off the edge of the table
check for rectus, iliacus, psoas major, and TFL
Ober’s Test
pt sidelying
PT supposts bent leg while stabilizing hip
allow the leg to drop
males should touch the table
females should be ~2in away from the table
MODIFIED: leg straight
Hamstring test
90/90 pt self supports legs in 90/90 position asked to straighten one leg at a time SLR PT lifts extended leg of pt. looking for other leg to rise up
Adductors
pt supine
ADd testing leg. if they cannot reach 40 deg, bend leg and see if it goes further. if it does, the long Add were restricting. if not, the short Add tight
Ely’s
pt prone
bend knee to stretch rectus femoris
is tight, pelvis will flex
also watch for any neurological signs on anterior thigh. may be indicative of femoral nerve syndrome
Quadrant test
Scour test
pt supine
PT flexed hip to 90 and gives compression force to hip while IR and ADd leg
tests for OA ofthe hip
FABER TEST
Pt supine
hip flex, ER, and ABd
post pain: SI
ant/groin pain: hip
FADIR test
pt supine
hip flex, Add, and IR
testing for femoral acetabular impingement
Craig test
testing for anteversion (15 deg) or retroversion (5 deg)
pt prone with knee flexed to 90 deg
PT IR and ER leg to feel for when the greater troch is at its most lateral position
hold in that position and measure the ankle of the leg
Fulcrum test
pt seated with legs hanging off table
Place towel roll under femur and press femur distal to the towel
pain may be from femur fracture
Patellar-pubis percussion test
pt supine
place stethescope over pubic symphasis
tap the patella, should here a sound if no fx
if sound if muffled, suspect fracture of the femur
