Piriformis Syndrome cluster (3)
- Pain at Sciatic N/Piriformis in side-lying FAIR
- TTP at Piriformis/Sciatic N
- SLR <65° or 15° asymmetry from unaffected side
FABER Procedure
Cross test leg over other leg, just above knee.
Push down to floor.
FABER (+)
Pain
Pain posterior = SIJ
Pain anterior = labrum
Scour Procedure
Supine, 90/90.
Axial load over knee.
Adduct hip, passively ER/IR.
Repeat with hip ABDucted.
Alternative: roll around in one big circle.
Scour (+)
(+) = reproduction of s/s, crepitus, roughness, “bumps”
FADIR Procedure
Flex hip >90.
PROM to end-range adduction & IR.
Third Test Procedure
Hip flex 90, adduct 10.
Part 1: axial load thru knee & IR.
Part 2: distraction & IR.
Third Test (+)
Pain in Part 1 (compression) and relief in Part 2 (distract)
Resisted ER Derotation Procedure
Supine or long-sitting
PROM hip flex & ER.
Pt actively moves into IR against therapist resistance.
Trendelenberg Procedure
SL stance on involved leg for 30sec.

unable to keep contra pelvis elevated
Murphy’s Percussion Procedure
Prone.
Find lowest ribs, heel of hand cover kidney region. Bump over top of flat hand.

Pain at kidney or hip
McBurney’s Point Procedure
Supine.
On R side, btwn ASIS & umbilicus.
Sink fingertips vertically down, hold for a second, then come off.
McBurney’s Point (+)
pain with pressure OR with recoil.
Check L side bc tenderness might be psoas

pain in anterior iliopsoas region, usually at 20-30°
