look at bone
alignment, lumbar lordosis, fixed flexion deformity of hip
look at gait
trendelenberg gait, antalagic gait, walking aids
feel skin
temp, distal sensation
feel soft tissue
swelling tenderness peripheral pulses
feel bone
greater trochanter tenderness [could be trochanteric bursitis]
move active and passive movement
modified Thomas test
Rotation medial and lateral
Abduction, Adduction
modified Thomas test
flexion 130, extension 0
in supine position flex the unaffected hip. if the affected hip rises from the plinth, this indicates loss of extension in the hip/ presence of fixed flexion deformity = positive Thomas test
Rotation medial and lateral
in supine position hip flexed at 90, knee flexed at 90
medial and lateral rotation 45
Abduction, Adduction
Abduction 45, Adduction 45
resisted movement
check hip muscle strength/ any pain on resisted movements of hip joint eg flexion extension adduction abduction medial and lateral rotation
Negative Trendelenberg test
Standing on normal leg only gluteus medius on supported leg contracts and elevates the pelvis on the unsupported side. elevation of the unsupported leg is normal indicating strong functional gluteus medius on the supported or standing side.
Positive Trendelenberg test
if the unsupported side drops, gluteus medius on standing leg may be weal or non-functioning
special hip test is called
