Hip Pain Differential (5)

A
  • OA - groin
  • Trochanter bursopathy - lateral, fluid-filled sac on trochanter; deep aching pain on lateral aspect of thigh esp when lie on it
  • Hip fracture - diff locations
  • Synovitis - more peds; usually transient but must r/o infection
  • Avascular necrosis - (called Legg-Calve- Perthes syndrome if in pediatric population) compromise blood supply to femoral head

Causes of Avascular Necrosis

A
  • PLASTIC RAGS
    • P- pancreatitis (alcohol)
    • L- lupus
    • A- alcohol
    • S - steroids
    • T- trauma
    • I - infection, idiopathic
    • C- collagen vascular disease, Caisson’s disease (decompression sickness - nitrogen in femoral head)
    • R- radiation
    • A- amyloid (deposits in vascular tissues of femoral head)
    • G - Goucher’s (lysosomal storage disorder - deposit tissue in abnormal places - hip)
    • S- sickle cell anemia (crisis in hip)
