What is DDH?

A

Congenital idiopathi Hip dysplasia

  • cogenital dislocation: supero-laterally
  • dysplastic acertabulum: hourglass deformity

Aetiology: Genetic ligament laxity (relaxin sensitivity) + mechanical factors in utero e.g breech + postnatal factors

Name two tests to help diagnose DDH?

  • ortolani: relocation on abduction
  • barlow: dislocation with adduction

Mx of DDH? Complications?

A
  • Dislocation: Treatment is cosmetic and prevents trandelenburg
  • dyspasia: prevents abnormal loading, subluxing and pain
    • < 6months: Pavlik Harness
    • 6-18 months: traction, closed reduction, spica,
    • > 18 months: open reduction, pelvic osteotomy: prevents OA
  • complication: AVN
