Hepatitis/Cholecystitis Flashcards
definition of hepatitis; etiology
inflammation of the liver, usually caused by viral infection, drugs, toxin, or parasites, which is characterized by diminished appetite, nausea, abdominal pain, jaundice and enlarged river.
Etiology: virus/drug/alcohol/parasites
Types A-E
Symptoms of Hepatitis: Acute Stage
flu like symptoms, light colored stool, yellowing o skin & eyes, abdominal pain, extreme fatigue, nausea, vomiting, dark urine
Symptoms of Hepatitis: Chronic Stage
malaise, weight loss, loss of appetite, jaundice, enlargement of liver
Types of Hepatitis:
- Yang Huang (Jaundice)
- Yin Huang (Jaundice)
Yang Huang: damp heat jaundice- bright yellow
Yin Huang: damp cold jaundice- light/dim yellow
Mechanism of Hepatitis
Damp and heat in the LV organ- 5 element disorder
- Liver–> overacting on SP
- Liver Insulting LU
- LV son-mom relationship with KD
- LV mom-son relationship with HT
Basic Points for Hepatitis
LV3, 8, 14; GB34; UB18, 19, DU9
Also:
DU14- drains heat
SP9; SP6- facilitate urination
UB20, ST36- strengthen SP to transform damp
UB34, SJ6, UB40, 17, LV2- add for chronic hepatitis
Damp Heat Retention
Acute
Yang Jaundice
- Acute Onset
- Chills, fever
- Digestive: poor appetite, gas/bloating; diarrhea or loose stools, nausea and vomiting
- muscular or joint pain
- jaundice (bright yellow), bitter taste, thirst
- distention of chest and hypochondria; irritability; headache
T: red with yellow greasy coat
P: slippery, wiry, may be fast
Basic + SJ5, LI11, SP9, ST40, GB43, ST44
LV Qi Stagnation (LV overacting on SP)
- Latent pathogen related dz
- bitter taste, nausea, decreased appetite, thirst
- dizziness
- emotional d/o: stress, depression, PMS, restlessness
- hyopchondriac pain & distention
T: normal with thin white coat
P: wiry
Basic + 4 gates, Ren 6; ST35, SP15; LV13, GB24
Excessive Toxic Heat
Acute
Hep A, E- may be fatal
Acute, fatal hepatitis with dyndrome of heat evil attacking the pericardia (may die within 2 weeks)
- high fever, yang jaundice
- thirsty, bitter taste; dark yellow urination; constipation
- shen d/o: delirium, Meniere’s, restlessness
- unconsciousness
T: scarlet with yellow brown coat
P: wiry, forceful, fast
Basic + UB40, SP10, DU14, P6
Spleen Qi Xu (Liver overacting on SP) [due to retention of dampness]
chronic onset with many digestive symptoms: poor appetite; gas/bloating in epigastric or abdominal region
- chest or abdominal distention
- vomiting; diarrhea or loose stools
- eating meat makes worse
- SP Qi Xu: fatigue, exhaustion, SOB; pale face, weakness of extremities, dizziness
- Jaundice
T: pale, may be swollen; white or white greasy coating
P: deep, weak
Basic+ ST36, SP6, R12, UB20, 21
Qi and Blood Stasis
liver cirrhosis and liver cancer patients (chronic hepatitis with syndromes of Qi stagnation and blood stasis & deficient SP Qi )
- enlarged liver and or spleen, or mass
- fixed, needling pain; worse at night
- distending and fullness, esp in the area of the liver
- breast fullness
T: purple, w/ purple spots and thin white coat
P: wiry, hesitant/choppy
Basic + SP10, UB40, UB17
Local and ashi points in liver area: LV14, GB22, 23, 24
LV & KD yin xu
chronic hepatitis or w/ liver cirrhosis, or in patients who take interferon therapy
- tenderness or dull pain; or burnign pain in the hypochondria area (liver)
- night sweats, palm heat
- thirsty, emaciation (liver yin xu)
- KD yin xu: low back pain, dizziness, vertigo
T: small, thin; may be dark red with peeled or no coat
P: thin, wiry; may be fast
Cholecystitis definition and biomedical mechanism
inflammation of the GB; bacterial infection/blockage/ bile secretion
Sx of Cholecystitis
+ Murphy’s sign
- upper right side abdominal pain
- biliary colic- spasmodic upper abdominal pain
- biliary colic after fatty meal
- abdominal discomfort
- pain under right shoulder blade
- fever, nausea, vomiting, flatulence, jaundice, itchy skin, pale stools
TCM mechanism for Cholecystitis
obstruction of GB channels d/t emotional disorders or irregular diet
