Hepatic/Hepatobiliary Disorders Flashcards
The liver consists of how many lobes? How many segments make up the lobes?
2 lobes and 8 segments
What are the 3 liver injury patterns?
- Hepatocellular
- Cholestatic
- Mixed
What labs indicated a hepatocellular injury pattern?
Disproportionate elevation in the serum aminotransferases (AST/ALT & Total/Direct Bilirubin) compared with Alk phos
What does DCD stand for?
Donation after Circulatory Death
What does DBD stand for?
Donation after Brain Death
What does MELD stand for?
Model for End-stage Liver Disease
What does LDLT stand for?
Living Donor Liver Transplantation
What does PBC stand for?
Primary Biliary Cholangitis
What does PSC stand for?
Primary Sclerosing Cholangitis
AST/ALT 2:1 is suspicious for?
Alcoholic liver disease
AST/ALT > 1000 only occurs in what 5 diagnoses?
- DILI, mc acetaminophen toxicity
- Ischemia/Shock liver (ischemic hepatitis
- Acute viral hepatitis (don’t forget HDV, A-E
- Autoimmune hepatitis
- Acute Budd-Chiari syndrome
even severe alcoholic injury shouldn’t cause AST/ALT greater than 300
Hyperkalemia treatment?
Calcium gluconate
Is psc or pbc associated with increased risk of bile duct cancer?
PSC
How is the diagnosis of PSC made?
Usually by MRI of bile ducts. Occasionally liver biopsy or ERCP is needed
How is the diagnosis of PBC made?
Two of the following:
1. Positive AMA (Antimitochondrial Antibidy)
2. Raised ALP (Alkaline phosphatase)
3. Diagnostic liver biopsy
