Why is this client receiving this medication?
-Prevention of thrombus formation.
How does this medication work?
- Potentiates the inhibitory effect of antithrombin on factor Xa and thrombin.
- Prevention of thrombus formation.
What side effects are most likely to occur secondary to this medication?
BLEEDING, HEPARIN-INDUCED THROMBOCYTOPENIA (HIT), anemia
What actions do I as a nurse need to take specific to this medication?
(Including adverse reactions I need to be aware of)
- Assess for signs of bleeding and hemorrhage.
- Assess patient for evidence of additional or increased thrombosis.
- Observe injection sites for hematomas, or inflammation.
- Monitor platelet count every 2–3 days throughout therapy.
- Have 2nd nurse check
What do I as a nurse need to teach the client about this medication?
- report any symptoms of unusual bleeding or bruising to health care provider immediately.
- not to take medications containing aspirin or NSAIDs while on heparin therapy.
- use a soft toothbrush and electric razor during heparin therapy.
What other medications is the client taking that may interact with/alter the action of this medication?
coumadin - increased risk of bleeding.
digoxin - may ↓ anticoagulant effect of heparin.
