Immune Thrombocytopenic Purpura
- AUTOIMMUNE production of IgG against platelet antigens
- antibody-bound platelets are destroyed by splenic macrophages
MUCOCUTANEOUS BLEEDING and low platelet count with normal peripheral smear and blood count (high megakaryocytic count)

Acute vs. Chronic
A
Acute- usually in children after a viral infection–usually goes away within a few weeks
Chronic- usually in young women
if secondary, usually associated with SLE, HIV, or Hep C

A
- steroids
- IV immunoglobulins
- splenectomy

A
thrombotic thrombocytopenic purpura
-deficiency in protease that cleaves vWF resulting in an accumulation of multimers, which causes activation and clumping of platelets
FARTN
- fever
- anemia (microangiopathic hemolytic anemia)
- renal failure
- thrombocytopenia
- neurologic changes

A
Dx: findings of hemolytic anemia with normal PT and PTT
Tx: plasmapheresis
