HIGH RBC
Primary: polycythemia vera
Secondary: causes with poor tissue oxygenation/hypoxia
Ix in reticulocytosis
serum EPO: differentiate b/w primary (low/normal) vs secondary (elevated) cause
?tumour secreting EPO - imaging
?PV - JAK 2 mutation
ferritin to rule out IDA
Rx of immune mediated neutropenia?
G-CSF (Filgrastim)
Causes of splenomegaly
CHINA Cirrhosis/congestion (potal HTN) Heme Infectious Neoplasm Autoimmune
Iron studies consist of?
Serum ferritin
Serum Iron (Fe)
TIBC
% saturation
Ix in IDA?
iron studies: low ferritin (low Fe, high TIBC, low % saturation)
blood film: hypochromic
Rx of IDA?
ferrous fumarate (continue until 3m after corrected) Hb normalise by 10g/L a wk
iron studies in anemia of chronic disease
Serum ferritin normal/increased
low serum iron/TIBC
What does the blood film show for sideroblastic anemia?
basophilic stippling, ring sideroblasts
iron granules in cells
