  1. Fever
  2. Recent or potential hemorrhage: check skin for raised or nonraised skin discoloration
  3. Thrombophelbitis: ask about blood clots
  4. Impaired sensation:“do you have normal feeling in this area?” “We have checked your sensation and it’s normal”
  5. Impaired cognition: check that pt is alert and oriented
  6. PVD (peripheral vascular disease): leads to impaired wound healing
  7. Devitalized tissue via Xray/ Radiation therapy (irradiated tissue)

  5. Edema: Determine chronic vs acute

  1. Cold hypersensitivity
  2. Cold intolerance
  3. Cryoglobulinemia: May present as rash or skin abnormalities secondary to other diseases such as Hep C
  4. Raynaud’s
  5. Paroxysmal cold hemoglobinuria: rare blood disorder in which the body’s immune system produces antibodies that destroy red blood cells when a person goes from cold to warm temperatures.
  6. Over regenerating peripheral nerves
  7. Over area with circulatory compromise or PVD

  1. Introduce self/ wash hands
  2. Prep pt (remove jewerly/ position pt safely)
  3. Sensation and Precautions: “Your skin looks good and your sensation is normal” “First let me ask you some health history questions: Do you have any history of heart issues? Do you currently have a fever? Do you have a history of blood clots? Do you have poor circulation? Are you pregnant?”
  4. Explain Treatment:“Now I will explain what you should expect to feel. This is a hot pack that should feel warm on your skin but not painful. If you feel pain at all let me know. This hot pack is good for increasing blood flow and relaxing the muscles and tissure before we stretch. Are you ok to proceed?”
  5. Inspect Skin: VERBALIZE
  6. “It should take a few moments to feel the warm on the skin; after I place it I will be back to check on you in 5 minutes”
  7. Apply hot pack under 8 layers of towels
  8. Check skin after 5 minutes
  9. Continue treatment 10 minutes and discard towels and put packs back in hydrocollator

  1. Introduce self/ wash handsI
  2. prep pt (remove jewelry/ position pt)
  3. Ask precautions/ contraindications:“Your skin looks good and your sensation is normal” “First let me ask you some health history questions: Do you have any history of heart issues? Do you currently have a fever? Do you have a history of blood clots? Do you know if you have a history of PVD? Do you have poor circulation? Are you pregnant?
  4. Explain Treatment: “We have checked your skin and sensation; both are intact. We are going to have you put your arm in this sleeve. When I turn on the machine, you feel small particles circulating around your arm and hand- these are cornhusks. It will be warm in there but should never be painful. If it ever is, please tell me. The fluidotherapy is good for warming up tissues and muscles and has the advatntage over the moist heat of adjusting the heat- also you can perform some hand ball exercises while in there. Are you ok to proceed?”
  5. Treatment: Inspect Skin AGAIN. VERBALIZE: “Skin is intact.” Secure sleeve. Turn machine on to 100-118*F. Adjust Agitation as tolerated by pt. Set for 10 minutes.
  6. SKIN CHECK 5 Min
  7. POST- Treatment: clean off arm with sleeve. Assess arm.

  2. prep pt (remove jewelry/ pt must wash hands/ position pt)
  3. Ask precautions/ contraindications:“Your skin looks good and your sensation is normal” “First let me ask you some _health history question_s: Do you have any history of heart issues? Do you currently have a fever? Do you have a history of blood clots? Do you know if you have a history of PVD? Do you have poor circulation? Are you pregnant?
  4. Explain Treatment:“We are going to have you dunk** your hand in the paraffin bath **6 times**. You will expect to feel warmth all over your hand right as your touch the paraffin. It will not burn you and it’s ok to touch the sides- but try to stay in the center and completely submerge your hand. This is really great for hands to warm up the muscles and joints of the fingers. And it just feels really good. **Are you ok to proceed?”
  5. Treatment: VERBALIZE skin check; have pt dunk hand. “if you feel any pain or discomfort, please tell me” Wrap in plastic bag. Seat pt comfortably; cover hand with towel. “I am going to check on you in 5 mins.” Check skin. Leave pt for 10 minutes. Then have pt peel off wax and discard.

  3. Ask Precautions/ Contraindications: We have checked this area for normal sensation; skin is intact and sensation is normal. Do you have any tumors in this area? Any known plastic or metal implants? Are you pregnant? Do you have a history of blood clots?
  4. Explain Treatment: We are going to put some gel on you and then I will use the US head over the affected area. This is good for heating up a very specific area with pain. You may or may not feel any heat, but you should never feel discomfort or pain. Please tell me if you feel pain. Are you ok to proceed?
  5. Inspect skin; VERBALIZE: skin intact.
  6. CHOOSE TREATMENT:
    1. Either: Thermal (cont) or Nonthermal (pulsed)
    2. Frequency: 1 (low) MHz: deep 5cm or 3 (high) MHz: 1-2 cm (PIPs, snuff box)
    3. Intensity: Thermal & deep choose: Higher 2.0w/cm2 for 8mins. Non-thermal choose: 0.5-1.0w/cm2
    4. Duration: the higher the intensity, the lower duration
  7. Apply Gel: Use circular motion; slowly
  8. Turn off and clean machine
  9. Assess skin
