RF of HF
CAD, smoking, MI, HTN, obesity, DM
Pathophys of HF
Pump failure –> fluid backup
- LHF –> lungs
- RHF –> systemic
How does pt in HF present?
Fatigue, SOB, orthopnea, nocturnal SOB, cough, wheezing, LE edema, nocturia
HF physical exam findings
Tachypnea, JVD, rales, wheezing, S3 gallop, edema, hepatomeg, PMI displacement
How to diagnose HF?
CXR, echo, BNP, ECG
How to treat HF?
- Stable: supportive–O2, morphine, diuretic, vasodilator
- HTN Crisis: O2, IV BB and nitro
- HypoTN: O2, inotrope or vasopressor
Diastolic HF
Normal EF and contraction
Systolic HF
Low EF and contraction
Class 1 HF
Mild. Heart disease but asympt
Class 2 HF
Mild. Heart disease, slight sx w/ ordinary activity
Class 3 HF
Moderate. Heart disease, sx w/ less than ordinary activity
Class 4 HF
Severe. Heart disease, can’t perform physical activity w/o sx or sx at rest
Stage A HF (and tx)
At risk for HF, no heart disease
Tx: diuretics, ACEI, ARB, lifestyle changes
Stage B HF (and tx)
Heart disease w/o sx
Tx: prevent progression (stage A tx) + BB in CAD
Stage C HF (and tx)
Heart disease w/ sx
Tx: (stage A tx) + wt monitor, compress socks
