What are the common signs and symptoms of HF
Peripheral edema hepatomegaly cardiomegaly reduced ejection fraction pulmonary edema
symptoms:
decreased exercise tolerance
shortness of breath
fatigue
What are dobutamine’s place in therapy?
acute decompensated heart failure
What is dobutamine’s MOA?
Beta-1 and Beta-2 activation
Increased force of contraction
increased heart rate
Counseling points for thiazide diuretics
avoid taking before bed
Monitor for ADEs especially related to electrolyte abnormalities
caution in patients with sulfa allergy
Counseling points for loop diuretics
avoid taking before bed
monitor urine output
watch for signs of dehydration
give IV slowly to avoid toxicity
What is Miltrinone’s MOA
Phosphodiesterase-3 inhibitor
increased cyclic AMP –> increased myocardial contractility
Vasodilation
What are Miltrinone’s adverse effects?
Arrythmias
Hypotension
