face/eyes

A

look at patients face and verbalize “i am assessing symmetry of the face, palpebral fissures, and nasolabial folds”

look at eyes and state “i am inspecting the eyes and sclera”

inspect conjunctive (ask pt to look up as they slide lids down)

state “i would dim the light” and assess both eyes for PERRL bilaterlly and assess both eyes for direct and consensual reflex

verabilize perrla stands for pupils are equal round and reactive to light and accomodation

assess for near reaction (convergence and constriction near gaze - accomodation)

verbalize “i am inspecting the teeth, upper and lower palaet, and bilateral areas under tongue using tongue blade to inspect tonsils

using tongue blade, inspect uvula: ask pt to say ahh

verbalize “i am observing the uvula rising symmetrically”
