verbalize “ I am doing a general survey assessing the patient’s dress and appearance, posture, facial expression, affect, LOC, hearing, speech, and orientation to time, place, person, and situation”
integumentary
verbalizes “throughout this assessment i will be inspecting the skin and noting any skin color changes, rashes, scars, or lesions. I will use Asymmetry Border Color Diameter and Evolving guidelines”
use dorsal surface of hands assessing skin temp bilaterly on upper and lower extremities
assess skin turgor under clavicle
face/eyes
look at patients face and verbalize “i am assessing symmetry of the face, palpebral fissures, and nasolabial folds”
look at eyes and state “i am inspecting the eyes and sclera”
inspect conjunctive (ask pt to look up as they slide lids down)
state “i would dim the light” and assess both eyes for PERRL bilaterlly and assess both eyes for direct and consensual reflex
verabilize perrla stands for pupils are equal round and reactive to light and accomodation
assess for near reaction (convergence and constriction near gaze - accomodation)
ears
inspect and palpate outer both ears; assess tragus for tenderness bilaterlly
intrsuct patient to put hand over left ear
perform whispher test (right side only)
nose/sinus
palpate frontal and maxillary sinus for tenderness
mouth/throat
apply gloves and use penlight to look in mouth
verbalize “i am inspecting the teeth, upper and lower palaet, and bilateral areas under tongue using tongue blade to inspect tonsils
using tongue blade, inspect uvula: ask pt to say ahh
verbalize “i am observing the uvula rising symmetrically”
neck (1)
verbalize “i am inspecting the trachea”
palpate trachea for midline position noting distance from SCM
palpate right and left thyroid gland lobes instructing pt to swallow as done
instruct pt to take a deep breath and hold while auscultating for carotid bruit
