Headaches Flashcards
A primary headache is
migraine, tension type, cluster
Secondary Headache are
caused by underlying organic disease
What is the preferred imaging to rule out hemorrhage
CT, if appears normal do a lumbar puncture
What imaging is for the posterior fossa
MRI
Onset of headache after 50 is a
red flag and consider secondary headache causation
Meningeal tests are
kernigs, and brudzinski’s
CN testing esp of
CN 2,3,4,6
Migraine pain is normally
unilater
tension HA normally
band like, bilateral
Cluster HA normally
STICTLY unilateral
Temporal arteritis
distrubition of temporal artery
Trigeminal neuralgia
distribution of trigeminal nerve
acute glaucoma
eye pain
Acute recurrent HA assoicated with vomiting, sleepiness, and pain free intervals Most likely
Migraine
Chronic progressive HA, with severity and frequency increased over time indicates
Structural disorder of CNS suspected
Chronic nonprogressive HA indicates
related to stress
vomiting without nausea indicates
increased intrcranial pressure
Epidural hematoma involves
arterieal involvement, it is biconvex and
epidural hematoma occurs betwen
the skull and dura
Subdural hematoma involves
venous, cresecent shaped
Subdural occurs between
dura and arachnoid
Unilateral throbbing headach preceded by an aura indicates
classic migraine
unilateral pain with pulsating quality for 4-72 hrs indicates
migraine
what is the MC types of migraine
one without an aura
tension type HA normally worse
in afternoon or early evening
aspirin and over the counter meds seem to relieve what HA
tension type
Tension type has one of two things but NOT both
photophobia or phonophobia
Cluster HA are strictly unilateral and
periorbital in 80% pf patients
lacrimation associated with a runny nose on the same side of HA indicates
Cluster HA
Cluster HA can make patients
pace, bang head against wall, or even attempt suicide
Temporal arteritis is a
immediate referral bc it can cause blindness
a sentinel HA precedes
stroke, of subarachnoid hemorrhage
What is the MC symptom of chiari malformation
HA
normal foramen magnum margins is
3mm
u suspect a skull fracture follow up is
CT
what is the MC skull fx
linear
