4 types of headaches to be able to differentiate
- cervicogenic HA (CPG “HA w/neck pain”)
- tension-type HA
- Migraine (with and without Aura)
- Cluster HA
What are the differences between Migraine with Aura vs. Migraine without Aura
-Aura - unilateral, aura (typically visual)
-Without Aura - unilateral, aggrevation with routine physical activity (like stairs), nausea
How many episodes of migraines must you have to be dx?
5 documented episodes
What is the best tx for migraines?
Not enough evidence that PT would address underlying cause. Should refer to neuro
How do we treat cluster HA?
Refer to appropriate provider (Oxygen and nasal triptan)
What does a cluster HA look like?
-severe unilateral pain behind the eye/temporal region/
-ipsilateral facial issues (ex: drooping eyelid, runny nose, swelling of eyelid)
-last for weeks or months (with periods of remisison)
- restlessness (supine makes worse)
-20-40y.o
-men>women
What type of headaches are unilateral?
-Migraines
-Cluster
-Cervicogenic
What type of headaches are bilateral?
Tension
What are the aggrevating factors for each type of headache?
- Tension = tenderness w/palpation (triggerpoints)
- Cervicogenic = pain with movement of neck & sustained postures
- Migraines = worse with physical activity (pts prefer rest)
- Cluster = restless and worse with supine
What type (severity) of pain do you have with the different type of headaches?
- Tension = mild or moderate “pressure/tightening”
- Migraine = moderate to severe “pulsating”
- Cluster = severe “sharp, burning, throbbing, tightening”
- Cervicogenic = Headache with neck pain “symptoms vary”
Length of time each type of headache lasts?
- Cervicogenic = position dependent
- Tension = minutes to days
- Migraines = 48-72hours
- Cluster = 15-180 minutes but can occur up to 8x/day
