GCS <13 on initial assessment GCS <15 2 hrs post trauma Suspected open or depressed skull fracture Evidence of basal skull fracture Post event seizure New focal neurology >1 episode of vomiting

Patient experienced LOC or amnesia since trauma + 1 of 4 criteria
Age >65
Bleeding/clotting disorders
Dangerous mechanism of injury
>30 mins retrograde amnesia from the event
Patient experienced LOC or amnesia since trauma + 1 of 4 criteria
CT head 8 hr
Patient experienced LOC or amnesia since trauma + Age >65
CT head 8 hr
Patient experienced LOC or amnesia since trauma + Bleeding/clotting disorders
CT head 8 hr
Patient experienced LOC or amnesia since trauma + Dangerous mechanism of injury
CT head 8 hr
Patient experienced LOC or amnesia since trauma + >30 mins retrograde amnesia from the event
CT head 8 hr
