Head And Neck Part 2 Flashcards
Preferred modality for thyroid gland
Ultrasound
It can be used to get high resolution images of thyroid gland
High frequency ultrasound
It provides better characterization of nodules
Ultrasound
It is a modality useful for large masses and to determine extent of intrathoracic extension
CT Scan
Contraindication in using contrast material in CT scan
If patient is for radioactive iodine treatment. (It will cause a delay in the treatment)
A classification system, based on breast lesions, used to facilitate characterization of whether a lesion is malignant or benign.
BIRADS
Factors used in characterization of lesion based on BIRADS
Shape Internal contents Echogenicity Halo Margins Calcification Doppler
If lesion grows across/along the plane of the lobe and is wider (or longer) than tall, is it benign or malignant? (Based on BIRADS)
BENIGN
If lesion grows on extra planes and it becomes taller than wide, is it benign or malignant?
MALIGNANT
Benign or malignant.
Internal contents are cystic
BENIGN
Benign or malignant.
Echogenicity maybe hypo, hyper, and isoechoic
BENIGN
Benign or malignant.
Echogenicity is markedly HYPOECHOIC or ISOECHOIC
MALIGNANT
Benign or malignant.
Halo is 60-80% with basket pattern
Benign
Benign or malignant.
Halo is 15%
MALIGNANT
Benign or malignant.
Margins are poorly defined, irregular, spiculated
MALIGNANT
Benign or malignant.
Margins are sharp, well-defined
Benign
Benign or malignant.
Calcification is egg shell, large, and coarse in appearance
Benign
Benign or malignant.
Calcification is interrupted eggshell in appearance with microcalcifications
MALIGNANT
Benign or malignant.
Using Doppler, it appears hyperplastic nodules- hypovascular
Benign
Benign or malignant.
In Doppler, well-differentiated thyroid CA- HYPERvascular with irregular tortuous vessels and AV shunting and anaplastic is HYPOvascular
MALIGNANT
What is TIRADS
Thyroid Imaging, Reporting, And Data System
- more points for MALIGNANCY, means higher grade of lesion
Criteria for malignancy (one point per criterion)
Clue: HI po! MAY PARTY IN my PLACE. THIS is the INVITATION CARD.
- HYPOECHOGENICITY
- Microcalcifications
- Partially cystic nodule with eccentric location of fluid portion and lobulation of solid component
- Irregular margins
- Perinodular thyroid parenchyma invasion
- Taller than wide
- Intranodular vascularity
- Cervical lymph node suspicious for malignancy
Thyroiditis can be diagnosed or followed-up with what modality?
