- Any recent illnesses or changes to lifestyle/ diet/ stress/ steroids that could’ve caused it

- What is diabetes?
- What is HbA1c?
- Why do we encourage HbA1c to be low?
- If results given, explain whether results are ideal or not

- How long they’ve had diabetes
- How well they think it’s being managed
- risk factor check (diet, exercise, BMI, HPT control)
- end organ damage/ symptom quick fire questions: 5 Ts, vision changes, recent TIA/ stroke/ heart attack, changes to urine and changes to sensation
How long, how well, risks, of end organ damage
What things need to be checked about patients medication regarding HbA1c 4
- How long they’ve been taking it
- adherence- if not, find out how regularly skipping, double dosing and why they’re struggling with adherence
- side effects/ adverse reactions
- how effective the medication has been at controlling their diabetes

- mood
- sleep
- diet
- ADL
- affecting work/ personal life
