List of 21 common TCM patterns
LV qi stagnation
mental depression, melancholy, moodiness, fluctuating, mental state, unhappiness, “feeling wound up”, irritability, sighing, chest fullness, distending or wandering pain in costal and hypochondriac regions, stuffiness in the chest, epigastric and abdominal distention and pain, “churning feeling in Stomach”, belching, poor appetite, borborygmus, diarrhea, possibly a foreign body sensation in the throat, difficulty swallowing, breast distention, irregular menstruation and dysmenorrhea, premenstrual tension and irritability, thin white coat with string-taut pulse
LV blood stasis
- prickling pain in hypochondriac regions or palpable mass, vomiting of blood, epistaxis, painful periods, irregular periods, menstrual blood dark and clotted, abdominal pain, “masses” in abdomen, purple nails & lips, purple complexion, dry skin (in severe case), petechiae, purple-dark color tongue, esp- on the sides, string-taut pulse
LV fire
- distended pain in head, temporal HA, dizziness and vertigo, redness, swelling, pain of eyes, tinnitus like the sound of waves, irritability, propensity to outburst of anger, dream disturbed sleep, thirst, bitter taste in the mouth, dryness in the mouth, intercostal/hypochondriac burning pain, dark-yellow urine and constipation with dry stools, hematemesis, hemoptysis, epistaxis, red tongue with yellow coating, string-taut and rapid pulse
internal wind from LV heat
- high fever, convulsion, tremor of limbs, neck rigidity, upward staring of eyes, in severe case, opisthotonus, coma and lock jaws, deep-red tongue with string-taut and rapid pulse
internal wind from yang rising
dizziness and vertigo, HA, numbness or tremor of limbs, convulsions, in severe case, collapse, coma, stiffness of tongue, dysphasia, aphasia or difficult speech, deviation of mouth and eyes and hemiplegia, red and tremulous tongue, string-taut and rapid pulse
LV damp-heat
- hypochondriac distention and pain, bitter taste in the mouth, poor appetite, nausea, vomiting, abdominal distention, scanty with dark-yellow urine,, there may be yellow sclera and skin of the entire body (jaundice), or fever, eczema of scrotum, swelling and burning pain in the testes, yellow foul leukorrhea with pruritus vulvae, yellow-sticky tongue coat and string-taut, rapid pulse
LV cold stagnation
lower abdominal (hypogastrium = just above the bladder) pain, distending pain with bearing down sensation in the testes, the scrotum may be contracted, in women there can be shrinking of vagina, paid is aggravated by cold and alleviated by warmth, white coating, rolling, deep, string-taut pulse
LV blood vacuity
- pallar, dizziness and vertigo, insomnia, blurry vision, “floaters in eyes”, dryness of eyes, night blindness, numbness of limbs, spasms of tendons, cramps, scanty menstrua flow of amenorrhea, withered and brittle nails, dry hair and skin, pale and thready pulse
LV yang rising from vacuity
HA is temples, eyes or lateral side of head, distending sensation of head, dizziness and vertigo, tinnitus, flushed face and red eyes, dry mouth and throat, irritability, insomnia and dream disturbed sleep, palpitation, feeling wounded-up, shouting in anger, poor memory, sore and weak lower back and knees, red tongue with string-taut, thready, and rapid pulse
GB deficiency
- dizziness, blurred vision, nervousness, timidity, propensity of being easily startled, lack of courage and initiative, sighing, pale or normal tongue color with weak pulse
HT qi vacuity
palpitation, S.O.B on exertion, spontaneous sweating, tiredness, listlessness, lassitude, pallor, pale tongue, thready, weak pulse or missed-beat pulse
HT yang - yang collapse
palpitation, S.O.B, feeble and shallow breathing, profuse sweating, cold limbs, cyanosis of lips, mental cloudiness or even coma, very pale or bluish purple and short tongue, feeble fading pulse
HT yang vacuity
- palpitation, S.O.B on exertion, spontaneous sweating, listlessness, lassitude, cold limbs, cyanosis of lips, pale, swollen and delicate tongue or a purplish dark tongue, weak, deep, or knotted pulse
HT blood vacuity
- palpitation, insomnia, dream-disturbed sleep, dizziness and vertigo, poor memory, pallor, pale lips, anxiety, tendency to be startled, pale, thin and slightly dry tongue, thready and weak pulse
HT yin vacuity
- palpitation, insomnia, dream-disturbed sleep, poor memory, mental restlessness, “uneasiness”, “fidgetiness”, heat sensation in palms and soles, tidal fever, malar flush, low grade fever in the evening, “feeling hot and bothered by heat”, night sweating, dry mouth and throat, 5-palm heat, red tongue with little or no coat, thready and rapid pulse
HT fire
- palpitation, mental restlessness, feeling agitated, “impulsiveness”, insomnia, flushed face, thirst, bitter taste, ulceration and pain in the mouth and tongue, hot and deep yellow urine, in severe case, hesitant and painful urination, dark urine or hematuria, red tongue with rapid pulse
Phlegm-Fire misting HT
derangement of mind, mania, incoherent speech, mental confusion, aggressive and violent behavior, tendency to hit and scold people, uncontrolled laughter and cry, shouting, muttering to oneself, palpitation, insomnia, dream-disturbed sleep, flushed face, coarse breathing, bitter taste in mouth, constipation, deep-yellow urine, in severe case, aphasia and coma, yellow sticky tongue coating, rolling, rapid and forceful pulse
Phlegm misting HT
- mental depression and dullness, mental confusion, lethargic stupor or incoherent speech, weeping and laughing without an appropriate reason, gurgling with sputum in throat, sudden collapse or coma, swollen tongue body with sticky white coating, string-taut and rolling pulse
HT blood stasis
palpitation, intermittent cardiac pain (stabbing or stuffy in nature in the precordial region or behind sternum) which often refers to the shoulder and arm, in severe case, may occur cyanosis of face, lips and nails, cold limbs and spontaneous sweating, purplish dark tongue or purplish dark spots, thready, hesitant or knotted pulse
SI qi pain
acute twisting pain of lower abdomen which may extend to back, abdominal distention, dislike pressure of abdomen, borborygmus, flatulence, abdominal pain relieved by emission of wind, bearing-down pain in the testes referring to the lumbar regione, white coat, deep and string-taut pulse
SI qi tied
sudden violent abdominal pain, dislike of pressure, abdominal distention, borborygmus, flatulence, vomiting, thick white coat, deep, string-taut pulse
