What is Dupuytren’s diesease?
Disease of the fascia in the palm & digits
What happens in Dupuytren’s disease
Palm fascia becomes thick & contracted. Cords & bands develop that extend into digits IV & V causing flexion deformities.
What are common medical treatments forDupuytren’s disease?
-non surgical injection
-Fasciotomy
-Aponeurotomy
-Fasciectomy (most common)
What function is impacted by Dupuytren’s disease?
Flexion-Gripping things
Extension-Releasing things
What are OT interventions for Dupuytren’s Disease?
-Wound care
-Edema control
-Scar Management
What are additional OT interventions for Dupuytren’s disease?
-Hand-based extension orthosis (dorsal or volar)
-AROM/PROM
progress to strengthening after 4 weeks when wounds healed
What is Skier’s Thumb?
Rupture of the ulnar collateral ligament of the MCP thumb joint
What are some OT interventions for Skier’s Thumb?
-Thumb orthosis for partial tear. Hand or forearm based; IP joint free
-AROM–>AAROM–>Lateral pinch strength
What function is impacted by Skier’s Thumb?
-Finger opposition
-Pinch strength
What are post operative treatments for Skiers Thumb?
-Thumb Orthosis
-Edema management
-Strengthening
What is complex regional pain syndrome (CRPS)?
-Vasomotor dysfunction as a result of an abnormal reflex
-Can be localized to a specific area or spread to other parts of the extremity
What are some OT interventions for CRPS?
-Pain Management
-Edema Management
-AROM to involved joints
-ADL; encourage pain free use
What are some additional OT interventions for CRPS (Complex Regional Pain Syndrome)
-Self management
-Orthotics to prevent contractures
-Weightbearing, joint distractions (scrubbing & carrying activities)
