- Amyloidosis and other infiltrative diseases

- Ischaemia

Causes of splenomegaly
A
Massive
- CML
- Myelofibrosis
- Primary lymphoma of the spleen, hairy cell leukaemia, malaria, kara-azar
Moderate
- Portal HTN
- Lymphoma
- Leukaemia (chronic or acute)
- Thalassemia
- Storage disease (eg Gaucher’s)
Small
- Other myeloproliferative - polycythaemia rubra vera, thrombocytaemia
- Haemolytic anaemia
- Megaloblastic anaemia
- Infection - viral (EBV, hepatitis), bacterial (IE)
- Connective tissue disease or vasculitis = RA, SLE, PAN
- Infiltration - amyloidosis, sarcoidosis

Causes of hepatosplenomegaly
A
- Chronic liver disease with portal hypertension
- Haematological disease - myeloproliferative, lymphoma, leukaemia, pernicious anaemia, sickle cell anaemia
- Infection - acute viral hepatitis, EBV, CMV
- Infiltration - amyloidosis, sarcoidosis
- Acromegaly
- Thyrotoxicosis

Causes of diffuse goitre
A
- Idiopathic
- Puberty, pregnancy, postpartum
- Graves disease
- Thyroiditis - Hashimoto’s thyroiditis, subacute thyroiditis (tender), chronic fibrosing thyroiditis
- Simple goitre - iodine deficiency
- Goitrogens - iodine excess, drugs
- Inborn errors of thyroid hormone synthesis - Pendred’s syndrome (AR disorder)
