Menstrual history (menarche, menopause, LMP, duration of menstruation, length of menstrual cycle, regularity, amount of bleeding, pain with period, IMB, PCB, problems with period)
Past gynae history (Pap smears, HPV vaccine, prev conditions/surgeries, STI history)
Sexual history (age first intercourse, number and gender of partners, sexual practices, libido, dyspareunia)
Contraceptive history (current, past, issues, compliance)
Past obstetric history (G/P, gestation, mode of delivery/management)
