Urinalysis
Microscopy and culture
Imaging if severely ill, persistent symptoms despite 2-3 days of antibiotics, suspected urinary tract obstruction, recurrence within a few weeks
Blood culture if septic

How to manage acute uncomplicated cystitis
A
Dipstick Drink a lot of fluid Empty bladder completely Wipe from front to back PCM for pain Sodium citrotartrate 4g orally 6hrly Trimethoprim 300mg OD for 3 days

A
Need to investigate to exclude underlying abnormality.
Dipstick, MC&S, U&E, ultrasound

How to manage acute pyelonephritis
A
If mild: use same dosage of trimethoprim but for 10 days. Or nitrofurantoin 100mg 6rly for 10 days.
If resistant: ciprofloxacin 500mg BD
If admitted: amoxicillin 2g IV 6hrly + gentamicin 4-6mg/kg/d IV, followed by oral therapy for 14 days
