Ferdinand’s guilt after killing the Duchess is so strong that he believes he is a wolf. This sudden change from sane to insane emphasises the effect that guilt can have on mental health.

Ironic corruption of his title; eventual impact of remorse and guilt; blasphemous; anguished soliloquy
Lack of faith in religion, and cyclical reference to the “devil” again - the Cardinal fulfils Bosola’s remark about him.
The idea of Fate and how the Cardinal fulfils it by the end
