Simple present passive
Am/is/ are + p.p.
Ex: Mary helps the boy.
Passive: The boy is helped by Mary.
Present progressive passive
am/is/are + being + p.p.
Ex: Mary is helping the boy.
Passive: The boy is being helped by Mary.
Present perfect passive
has been + p.p.
Ex: Mary has helped the boy.
Passive: The boy has been helped by Mary.
Simple past passive
was/were + p.p.
Ex: Mary helped the boy.
Passive: The boy was helped by Mary.
Past progressive passive
was/were being + p.p.
Ex: Mary was helping the boy.
Passive: The boy was being helped by Mary.
Past perfect passive
had been + p.p.
Ex: Mary had helped the boy.

Simple future passive
will be + p.p.

be going to passive
am/is/are going to be + p.p.
