Gradual Blurry Vision Flashcards
Common causes of gradual vision loss
SPK EBMD Refractive error shift AMD NS
NS
Most common type of cataract with aging.
Myopic shift that can be compensated for early on
What kind of refractive shift for NS
Myopic
What kind of refractive shift for cortical cats
Hyperopic
PSC
Steroids and X-rays
Mild central PSCs can significantly reduce vision (near more than distance)
More severe glare
What grade is considered significant
2+
EBMD and vision loss
Can result in vision loss if the basement membrane changes are located within the central cornea, or if an associated RCE occurs (10% of cases). They are usually asymptomatic
- most common anterior corneal dystrophy
- AD
- more in females
- bialteral, non progressive
- abnormal corneal epithelial adhesion and excessive basement membrane production
- negative staining
EBMD and RCE
10% of patients with EBMD will have RCE and 50% of patients with RCE have EBMD
Posterior blepharitis and MGD
Used interchangeably, but should not be. Posterior blepharitis is a general term that refers to inflammatory conditions of the eyelid, and may be caused by several conditions. In the later stages of MGD, if inflammatory signs are present, an MGD-related posterior blepharitis is the appropriate diagnosis
Things that can cause presenile cataracts
Wilson is MAD
- Wilson’s
- myotonic dystrophy
- atopic dermatitis
- DM
Things associated with infantile cataracts
Rubella and galactosemia
-lamellar is the most common
Drugs that cause whirl K
Fabry swirls his CHAI-T
- Fabrys
- chloroquine
- hydroxychloroquine
- amiodarone
- indomethacin
- tamoxifen
Plaquenil exam sequence
Baseline exam within 1 year if starting
Annual after 5y (10-2 and SD-OCT)
What is the typical dose for plaquenil
5mg/kg/day
135lbs cut off for 200/400mg
What are the high risks for plaquenil
TX>5 years Decreased renal/liver function >60 Increased body fat Concomitant retinal disease
