Colchicine
inhibits migration/activation leukocytes. binds tubulin, interferes with function of mitotic spindles. first line, non analgesic, not anti-inflammatory in other disorders. used w/ caution in patients w/ hepatic/renal/CV disease. not for preggers. adverse GI irritation from oral, skin necrosis from IV
Allopurinol, Febuxostat
inhibit xanthine oxidase, uric acid synthesis. does not affect acute attack. can be given to compromised renal function, aspirin takers. adverse rash, GI upset
Uricosuric agent Sulfinpyrazone
inhibit urate reabsorption. ineffective in aspirin takers. depends on renal function. may cause initial acute attack. does not affect acute attacks. not for patients with blood abnormalities, kidney stones. adverse GI upset, inhibited platelet function, not for hypersensitivity to phenylbutazone
Uricosuric agent Probenacid
inhibit urate reabsorption. ineffective in aspirin takers. depends on renal function. may cause initial acute attack. does not affect acute attacks. not for patients with blood abnormalities, kidney stones. adverse GI upset, not for those w/ ulcers, some develop hypersensitivity, overdose CNS issues+death
