What is Gout?
Hyperuricemia (high uric acid in blood)
Sodium Urate crystal deposition along joints + interarticularly
Who is a typical Px?
Middle aged overweight men
What are the risk factors gout?
Purine rich food, meat, seafood, Beer, fizzy drinks
CKD + diuretics
What food can be anti gout?
dairy
what is the pathology of the crystals forming?
Purines -(Xanthine oxidase)-> Uric Acid <–> Monosodium urate (sharp crystals)
Uric acid –> Kidney excretion also
High uric acid/CKD =
impaired excretion therefore high monosodium urate
Symptoms of gout
Monoarticular; typically big toe (metatarsalphalangeal joint), sudden onset, severe, swollen red toe, can’t put weight on it (*bed sheets)
What is a differential diagnosis?
septic arthritis
Diagnosis of gout
Tests and results
Joint aspirate + polarised light microscopy
Negatively vely birefringement needle shaped crystals
Treatment for gout
Diet; decrease purines, increase dairy
NSAIDS, colchicine then consider steroid injections
Prevention (lower uric acid levels) = ALLOPURINOL
What is colchicine?
Anti inflammatory - inhibit WBC migration
What is the mechanism of allopurinol?
Xanthine oxidase inhibitor (lower uric acid)
What is pseudo gout?
Calcium pyrophosphate crystals deposit along joint capsule
