inflammation of a joint caused by uric acid crystals either by excess production or under excretion

- raised inflammatory markers and uric acid level (can be normal during acute attack)
- aspiration of joint fluid (negative birefringent, needle-shaped urate crystals)

A
acute= NSAIDS, colchicine and steroids prophylaxis= allopurinol

when should start allopurinol in gout?
A
start 2-4 weeks after acute attacks as risks exacerbating gout

define pseudo-gout
A
this is inflammation of a joint cause by calcium pyrophosphate/hydroxyapatite crystals

diagnosis of pseudo-gut
A
- envelope shaped/rhomboid, mildly positive birefringent Ca pyrophosphate crystals
- XR shows chondrocalcinosis

A
NSAIDS
colchicine
