Diagnosis of gout
Monosodium urate crystals in joint fluid
Crystals are needle shaped and negatively birefringent crystals
Features of acute gout
Monoarticular arthritis of first MTP or tarsal joint
If it presents at first MTP(podagra), joint aspirate not required
Transplantation gout
Associated with use of cyclosporine(calcineuron antagonist)
Lifestyle modifications to treat gout
Purine restriction, weight loss, discontinue etoh,
D/c thiazides diuretics, low dose salicylates
Acute flair treatment
-1st line: NSAIDs, use steroids if NSAIDs not safe
Joint injection meds ineffective
2 or more attacks per year or presence of tophi or kidney stones
Start allopurinol(>300mg/day), goal serum Uric acid
3 stages of gout
- a symptomatic hyperuricemia: lasts several decades
- acute intermittent gout
- chronic tophaceous gout
