- fhx
- drugs (diuretics, aspirin)
- comorbidities: renal disease, obesity, alcohol
- metabolic syndrom
patho of gout
uric acid is precipitated out into uric acid crystals in the joint space ➔ inflammatory response ➔ swells up and stretches the capsule ➔ red and painful
patho of tophi formation
aggregates of MSU form and granulomatous tissue surround it in the case of chronic gout
ix for gout
synovial fluid analysis – test for crystals (birefringent)
CBC and CRP - may have elevated WBC and elevated CRP bc inflammation
serum uric acid
tx for gout
acute
- colchicine
- NSAIDs - ibuprofen
- intraarticular steroid injections
chronic
- allopurinol (xanthine oxidase inhibitor)
- NSAIDs prn
- wt loss
- pt education of trigger avoidance
gout triggers
red meat
fish
organ meat
alcohol
high fructose drinks
dehydration/starvation/trauma
drugs: ASA or diuretics
