Heartburn
Reflux
Retrosternal pain
Sometimes cough
NB RED FLAGS

What are the causes of GORD?
A
- delayed gastric emptying
- hiatus hernia
- Poor LOS
- Poor peristaltic action

What are the investigations for GORD?
A
If <55 and no red flags then based on clincal findings
if >55 or red flags then endoscopy

What are the investigations for a suspected hiatus hernia?
A
Barium swallow

What is the test for measuring reflux?
A
24 hr nasogastric cannula?

What is Barretts oesophagus and why is it dangerous?
A
Continual reflux causing dysplasia and then metaplasia of striated to columnar epithelium
Can lead to adenocarcinoma

What is the 1st line treatment?
A
Advice, stop meds and NSAIDS

What is the 2nd line treatment?
A
PPIs
Ranitidine - h2 antagonist
omeprazole - blocks parietal cells
metaclopramide - increases gastric emptying
