When should a glucose curve be performed after a dose change/starting therapy?
No earlier than 7-10 days
What should be performed as routine monitoring of diabetic animals when stable?
Clinical signs
Ideally home BGC (guidelines recommend BGC)
Fructosamine
What things should be looked at when analysing the glucose curve?
Duration of insulin action
Time to peak activity
Time in the target range (4.5-14mmol/l for cats)
Peak BGCs (fasting)
IS hypoglycaemia identified on a glucose curve always significant?
Yes!
How can the insulin dose be adjusted if the nadir is <4.5mmol/l?
Reduce dose by 50% or consider longer acting preparation if peak BG is >14mmol/l
How can the insulin dose be adjusted if the nadir is 4.5-14mmol/l?
Continue on current therapy
How can the insulin dose be adjusted if the nadir is 4.5-8mmol/l and the peak is >14mmol/l?
Maintain therapy (if at beginning) and re-test in 1-2w. Change to longer acting Increase dose by 0.5u/cat.
How can the insulin dose be adjusted if the nadir is >8 and peak >14 and there are clinical signs?
Increase dose by 0.5-1iu/cat.
What is the feline BGC target range?
4.5-14mmol/l
What is the anticipated nadir & DoA of lente insulin (can insulin) in cats?
Nadir: 2-8h
DoA: 8-10h
What is the anticipated nadir & DoA of PZI (ProZinc) in cats?
Nadir: 2-6h
