GI Pharmacology Flashcards
What are the different classes of anti-emetics?
Antihistamines
D2 antagonists
Antimuscarinics
5HT antagonists
MOA of antihistamines in vomiting
Block H1 receptors in vomiting centre
Example of antihistamines for vomiting and dose
Cyclizine
Promethazine
Cyclizine, 50mg IM/IV/oral every 8 hours
Give some side effects of antihistamines
Sedation
Antimuscarinic - dry mouth, blurred vision
Hypotension
MOA of antimuscarinics
Blocks action of ACh, which stimulates vomiting via the vomiting centre
Indications for antimuscarinics?
Motion sickness
- can be given as patch
Side effects of antimuscarinic
Increased body temp Mydriasis Dry mouth Dry eyes Decreased sweating Flushed face Delirium
MOA of dopamine receptor antagonists
Block action of dopamine at the chemoreceptor trigger zone
What are the different types of dopamine receptor antagonists?
Haloperidol
Chlorpromazine
Metoclopramide
Domperidone
When are dopamine receptor antagonists used for?
Sickness due to drugs, cytotoxics and diffuse malignancy
Give the MOA of metoclopramide
D2 antagonist plus increasing gut motility adn increasing gastro-oesophageal sphincter tone.
Give the dose of metclopramide
10 mg, 8 hourly
IV/IM/oral
What is the best antiemetic for visceral causes of nausea?
Metclopramide
MOA for 5HT receptor antagonists
Block the 5HT receptor in the CTZ and the gut.
Give some examples of 5HT receptor antagonists
Ondansetron
Granisetron
