GERD
Desquamation, elongated submucosal pegs, inflammatory cells
Barrett Esophagus
Columnar with goblet cells + hyperchromatic nuclei, less goblet cells (low-grade dysplasia) + nuclear atypia, irregularity (high-grade dysplasia) + invasive (adenocarcinoma)
Acute hemorrhagic gastritis
Numerous petechiae, erosions, ulcers, bigger hemorrhages
Chronic gastritis
Atrophy, metaplasia, dysplasia, neoplasia, H. pylori (antrum), germinal centers
Peptic ulcer disease
Small, round, deep, + punched out
Chronic peptic ulcer
Necrosis, inflammation, granulation tissue, scar
Benign ulcer
Round-oval, flat or overhanging margins, deep punched-out, smooth
Malignant ulcer
Irregular shape, heaped-up margins, shallow sloping, necrotic shaggy
Stress ulcer
Deep, single, hemorrhagic, small (< 1 cm)
Esophageal squamous cell carcinoma
Aggregate with eosinophilic cytoplasm, keratin pearl, trebeculae with cell cluster around keratin pearl
Types of esophageal squamous cell carcinoma
Fungating obstructive polypoid mass, diffuse circumferential thickening (napkin-ring), numerous coalescing nodules, stricture w/ hemorrhagic ulcer
Gastric polyps - hyperplastic
Asymptomatic, rarely malignant, cystically dilated glands, inflamed edematous lamina propia
Gastric polyps - fundic gland
< 1 cm, fundic, cystically dilated glands, oxyntic cells
Gastric polyps - adenomatous
Antrum, dysplasia, crowded, hyperchromatic, glands within glands
Gastric adenocarcinoma
Thickened wall, linitis plastica
