How is a H.Pylori infection usually treated with

A
  • Without penicillin allergy: PPI, amoxicillin and either clarithromycin or metronidazole
  • With penicillin allergy: PPI, clarithromycin and metronidazole.
  • Need to consider previous treatment with antibiotics

What are contraindications and side effects of Clarithromycin

A
  • dizziness, GI discomfort, dry mouth.
  • Contraindcation with DOACs (monitor side effects), CCB (monitor+adjust), statin (avoid/adjust rhabdomyolysis), Corticosteroid (monitor adverse effects), salbutamol (avoid)

What are some councelling points for Clarithromycin?

A
  • Make sure to take the complete the treatment as prescribed. Antibiotic resistance and return if no improvement in symptoms in 3 days
  • Can take with food
  • Alcohol in moderation unlikely to cause harm

A
  • Dont drink alcohol until 48hrs after treatment
  • Side effects: nausea, vomiting, GI discomfort
  • Contraindication with warfarin - monitor INR + adjust dose

A
  • Take with or after food with a full glass of water and don’t drink alcohol until 48hrs after stopping
  • Make sure to take full course of treatment and return if no improvement in 3 days

A
  • Do not give with penicillin allergy
  • Contraindicated with warfarin (monitor INR and adjust dose)
  • Side Effects: Diarrhoea, nausea, skin reactions

What are some counselling points for Ibuprofen

A
  • If any sign of stomach discomfort/pain. Ask about stools and bleeding. Refer + stop ibuprofen
  • Should take a lowest dose for the shortest duration to avoid side effects
  • Take with a meal - less likely for side effects + avoid lying down after taking
